WELCOME

We are excited and extremely grateful that you have decided to join 1 Nation AAU!

The following electronic sign-up form contains a student athlete / parent contract that details our expectations for each of our athletes, a waiver release form, social media Waiver/Release Form, and a Waiver/Release for Communicable Diseases including COVID-19, all of which needs to be confirmed below in order for your athlete to fully participate in our program.

Again, thank you for choosing 1 Nation AAU and allowing us the PRIVILEGE of continuing your athlete’s development!

Tuition amount varies from team to team, please reach out to Staff to get the tuition amount for your athlete.

A Registration / Uniform Fee in the amount of $300.00 ( included in total tuition ) will be due ASAP after electronically filling AND signing this form.
( unless already paid )
If you would like to break the remaining balance into four (4) smaller participation payments, the first payment will be due no later than March 1st, 2024, then monthly on the 1st until your balance is paid in full.
There will be an additional $25.00 Fee assessed for payments made after the five (5) day grace period. ( the 6th of the month )

Communication with staff is STRONGLY encouraged.

Please fill and review the following 1 Nation AAU NEW ATHLETE REGISTRATION form below. Please acknowledge each waiver by clicking the check box below each section.

Once you have read, filled, and submitted the below form, you will immediately be emailed a link from 1 Nation’s DocuSign account to digitally sign the form to COMPLETE YOUR ATHLETES REGISTRATION.

Registration is NOT complete until you have digitally signed this agreement using DocuSign. ( no cost )

This is an electronic form, and the parent / legal guardian will need to sign electronically before moving forward in our program.

IF YOU DO NOT RESPOND TO THE DOCUSIGN EMAIL REQUESTING SIGNATURE, YOUR ATHLETE SIGN-UP FORM IS DEEMED INCOMPLETE, AND YOUR ATHLETE WILL NOT BE ABLE TO PRACTICE / PLAY UNTIL 1 NATION AAU HAS THE PARENT OR LEGAL GUARDIAN SIGNATURE ON THE ELECTRONIC SIGN-UP FORM SENT FROM 1 NATION’S DOCUSIGN ACCOUNT.
